Course Change Form 2007/8

 

The course selection and course change process requires careful attention. Please follow the procedure outlined below. Once appropriate signatures have been received, the change may be processed. You are to remain in your class until the process is complete. Text books must be returned to the library.

 

Please be aware that all courses dropped after mid-term will be included on the final transcript. Students are not permitted to add courses fifteen classes into a semester. By signing below, the student acknowledges responsibility for materials already covered in class, and recognizes that assignments and evaluation may be determined in consultation with the subject teacher.

Students who add a class between 5 and 15 days into the semester are required to complete the following in consultation with the teacher. Students have one week after acceptance into a class to complete required tasks and submit the completed form to the Student Services office. It is the student’s responsibility to ensure the following are complete. Students who do not complete all tasks by the date assigned will be expected to attend the next available after school assistance session.
